Jake Nelsen is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) who provides care for individuals across the lifespan. A long time Coloradan, he completed a Bachelor of Arts in Psychology at the University of Colorado Boulder, a Bachelor of Science in Nursing at the University of Colorado Anschutz Medical Campus, and a Master of Science in Nursing at Vanderbilt University.

Jake’s career has included working with people experiencing homelessness as well as providing nursing care for patients in inpatient psychiatric settings. These experiences have given him a deep respect for the challenges people face and the importance of addressing mental health within the context of each person’s life. He is committed to forming genuine partnerships with patients, developing treatment plans that reflect their goals and values, and using evidence-based practice to integrate therapy, lifestyle approaches, and medications.

When he’s not working, Jake enjoys being with his family, reading, mountain biking, and backcountry skiing.
